Is acupuncture an alternative for the treatment of Parkinson’s Disease?

Resumen

INTRODUCCIÓN
Se postula que la acupuntura podría tener diversos beneficios para pacientes con enfermedad de Parkinson. No obstante, su real efectividad clínica aún es discutida.

MÉTODOS
Para responder esta pregunta utilizamos Epistemonikos, la mayor base de datos de revisiones sistemáticas en salud, la cual es mantenida mediante búsquedas en múltiples fuentes de información, incluyendo MEDLINE, EMBASE, Cochrane, entre otras. Extrajimos los datos desde las revisiones identificadas, reanalizamos los datos de los estudios primarios, realizamos un metanálisis y preparamos una tabla de resumen de los resultados utilizando el método GRADE.

RESULTADOS Y CONCLUSIONES
Identificamos nueve revisiones sistemáticas que en conjunto incluyen 53 estudios primarios, de los cuales 45 corresponden a ensayos aleatorizados. Concluimos que la acupuntura podría tener un efecto mínimo en mejorar los síntomas motores y la discapacidad en la enfermedad de Parkinson, pero la certeza de la evidencia es baja.


 
Problema

La enfermedad de Parkinson es una enfermedad neurodegenerativa crónica frecuente. El tratamiento de elección es la levodopa, pero a largo plazo su efectividad disminuye y pueden aparecer efectos adversos como deterioro de fin de dosis y discinesias. En este contexto, muchos pacientes buscan terapias complementarias, entre ellas la acupuntura, la cual se cree ejercería efectos sistémicos por medio de su acción local. De esta forma lograría una mejoría sobre los síntomas motores del Parkinson y el grado de discapacidad. No obstante, aún no está claro cuál es su real efecto clínico en esta enfermedad.

Métodos

Para responder esta pregunta utilizamos Epistemonikos, la mayor base de datos de revisiones sistemáticas en salud, la cual es mantenida mediante búsquedas en múltiples fuentes de información, incluyendo MEDLINE, EMBASE, Cochrane, entre otras. Extrajimos los datos desde las revisiones identificadas y reanalizamos los  datos de los estudios primarios. Con esta información, generamos un resumen estructurado denominado FRISBEE (Friendly Summaries of Body of Evidence using Epistemonikos), siguiendo un formato preestablecido, que incluye mensajes clave, un resumen del conjunto de evidencia (presentado como matriz de evidencia en Epistemonikos), metanálisis del total de los estudios cuando sea posible, una tabla de resumen de resultados con el método GRADE y una sección de otras consideraciones para la toma de decisión.

Qué tipo de pacientes incluyeron los estudios*

Se incluyeron pacientes con enfermedad de Parkinson con o sin complicaciones motoras. No obstante, no se detalla el grado de progresión de la enfermedad, ni la proporción de pacientes con compromiso cognitivo y otras manifestaciones no motoras.

Qué tipo de intervenciones incluyeron los estudios*

En 16 ensayos se utilizó acupuntura tradicional [18],[19],[21],[23],
[27],[30],[35],[36],[37],[38],[39],[40],[41],[47],[48],[49],[60], en nueve electroacupuntura [22],[24],[28],[32],[36],[43],[45],[59],[62],
en 18  acupuntura de cuero cabelludo [25],[26],[29],[31],[33],[34],
[42],[44],[50],[51],[52],[53],[54],[55],[56],[57],[58],[61] y en dos acupuntura abdominal [20],[46].

En 33 ensayos se comparó contra no uso de acupuntura [19],[20],

[43],[46],[48],[49],[50],[51],[52],[53],[54],[55],[56],[57],[58],[59],
[60],[61],[62], en siete contra medicamentos [27],[32],[37],[38],[44],[45],[47] y en cinco contra acupuntura simulada [18],[24],[35],[36],[40].

Qué tipo de desenlaces midieron

De los múltiples desenlaces medidos por los ensayos, las revisiones sistemáticas presentaron de manera agrupada los siguientes:

  • Estado motor evaluado mediante UPDRS (Unified Parkinson’s Disease Rating Scale)
  • Grado de discapacidad, evaluado mediante la escala de Webster 
  • Efectos adversos 
  • Constipación
  • Disfunción vesical

El seguimiento fluctuó entre 4 semanas y 3 meses.

Resumen de los resultados

La información sobre los efectos de la acupuntura sobre los síntomas motores está basada en seis ensayos aleatorizados [21],[29],[31],[34],[41],[59] que incluyen 396 pacientes. El resto de los ensayos no reportaron los desenlaces de interés o lo hicieron de una forma que no permitió su incorporación en el metanálisis.

Todos los ensayos reportaron síntomas motores medidos en escala UPDRS III (396 pacientes). Cuatro ensayos [28],[34],[61],[62] reportaron grado de discapacidad medido en Webster (208 pacientes) y cuatro ensayos [23],[24],[44],[55] reportaron el desenlace efectos adversos (150 pacientes).

El resumen de los resultados es el siguiente:

  • La acupuntura podría tener un efecto mínimo en mejorar los síntomas motores en la enfermedad de Parkinson, pero la certeza de la evidencia es baja.
  • La acupuntura podría tener un efecto mínimo en mejorar la discapacidad en la enfermedad de Parkinson, pero la certeza de la evidencia es baja.
  • La acupuntura probablemente se asocia a efectos adversos leves y poco frecuentes. La certeza de la evidencia es moderada.

Otras consideraciones para la toma de decisión

A quién se aplica y a quién no se aplica esta evidencia
  • La evidencia presentada en este resumen se aplica de manera amplia a cualquier paciente con enfermedad de Parkinson.
  • Las limitaciones de la evidencia existente no permiten establecer si existe algún subconjunto de pacientes en los que pudiera existir un efecto diferente.
  • Cabe destacar que, en las diferentes revisiones, no se detalla el grado de progresión de la enfermedad, ni la proporción de pacientes con compromiso cognitivo y otras manifestaciones no motoras.
  • Es importante tener presente que no se pudo incorporar en la tabla de resumen a ninguno de los ensayos que comparan acupuntura versus acupuntura placebo o simulada, que es la comparación más relevante para determinar el efecto genuino de la acupuntura. Las comparaciones versus no tratamiento no logran determinar la diferencia entre el efecto genuino de la acupuntura y el efecto ligado al ritual asociado a esta práctica.
Sobre los desenlaces incluidos en este resumen
  • Los desenlaces presentados en la tabla de resumen de resultados son aquellos considerados críticos para la toma de decisión por los autores de este artículo y en general coinciden con lo reportado por las revisiones sistemáticas.
  • Por la naturaleza de la intervención, son múltiples los desenlaces que pudieran verse afectados, como velocidad de progresión de enfermedad, requerimientos de medicamentos, UPDRS total, pero sobre ninguno de ellos se observó un efecto que podría modificar sustantivamente lo planteado en este resumen.
Balance riesgo/beneficio y certeza de la evidencia
  • La certeza de la evidencia es baja, pero si el efecto observado fuera real se trataría de un beneficio de mínima cuantía.
  • La evidencia existente no permite determinar si existe algún tipo de acupuntura (por ej. protocolos estandarizados de acupuntura para la enfermedad de Parkinson) que podría tener algún beneficio clínicamente relevante.
Consideraciones de recursos
  • Se trata de una intervención con un costo relativamente alto. Si bien no es posible realizar un adecuado balance entre beneficios y costos por la incertidumbre asociada a los primeros, es muy probable que no se trate de una intervención costoefectiva.
Qué piensan los pacientes y sus tratantes
  • Frente a la evidencia presentada en este resumen, la mayoría de los pacientes y tratantes debiera inclinarse en contra de la utilización de la acupuntura en la enfermedad de Parkinson.
  • Sin embargo, dado que existe una percepción positiva sobre los efectos terapéuticos de la acupuntura, tanto en la ciudadanía como en muchos profesionales de salud, es razonable esperar variabilidad en la toma de decisiones.
Diferencias entre este resumen y otras fuentes
  • Las conclusiones de este resumen difieren parcialmente de las de las revisiones sistemáticas identificadas, las cuales refieren que probablemente existe un efecto beneficioso, aunque concuerdan en relación a las limitaciones de la evidencia existente.
  • Las principales guías clínicas, como la guía de la Movement Disorders Society [63] y Sociedad Canadiense [64], mencionan a la acupuntura dentro de las medidas no farmacológicas, pero refieren que falta evidencia para determinar si tiene algún efecto clínico para la mejoría de síntomas motores, y que se trata de una terapia en etapa de investigación. 
¿Puede que cambie esta información en el futuro?
  • La probabilidad de que futura evidencia cambie las conclusiones de este resumen sobre los beneficios de la acupuntura en enfermedad de Parkinson es alta, debido a la baja certeza de la evidencia existente.
  • Según la International Clinical Trials Registry Platform de la Organización Mundial de la Salud, existen al menos 9 ensayos en curso [65],[66],[67],[68],[69],[70],[71],[72],[73], los cuales podrían aportar información relevante.
